Background
The TB burden in Indonesia ranks the second highest in the world (WHO, 2020) and TB remains the leading cause of death by communicable diseases in Indonesia (IHME, 2019). There were 98,000 people who died due to TB in 2018 (WHO, 2019). Although there has been a 19% decline in mortality from 2007 to 2017, TB remains a major health challenge in Indonesia. The Government of Indonesia has adopted the global commitment to end TB by 2030, and now must decide what to prioritize and how to deliver. From 2017 – 2020 the Center for Financing and Health Insurance (PPJK) of Ministry of Health (MoH) led analytical works of multi-stakeholders (Tim Perumus) including development of TB Strategic Health Purchasing (SHP) pilot design.
The Tim Perumus outlined priority service delivery objectives for TB and identified health financing bottlenecks to achieving those objectives. The SHP team supported analysis, formulation and validation of options, and developed recommendations for achieving better TB outcomes using more strategic health purchasing. After finalizing the recommendations, the SHP team looks forward to conducting pilot preparation and implementation in the selected districts from 2021 to 2023. During the design and implementation preparation (ToT on SHP governance manual) period, an observation will be conducted to capture stakeholder concerns and respond to them. It includes gathering insights for the further implementation arrangement.
The Director of Communicable Diseases of the Ministry of Health had selected two pilot districts for TB SHP which are Kota Medan, North Sumatra and Kota Denpasar, Bali. Pilot implementation in Medan began in second quarter of 2021. Adjustment to the contracting arrangements are expected to come first at the third quarter of 2021, followed by the implementation of new payment arrangements in 2022. Pilot preparation in Denpasar started from the first quarter of 2021 and will be implemented starting from the end of the year 2021.
The design and implementation of TB SHP are supported by two USAID projects – The Health Financing Activity and the TB for Private Sector (TBPS) project. The Health Financing Activity is responsible for leading design, national coordination and analytical works, and providing technical input to the development of modules related to capacity building and advocacy in regional level. TBPS is responsible for initial assessment at district level, district coordination and facilitation of the SHP pilot process at district level, including personnel support, data collection, and coordination with district authorities related to SHP works.
The Technical Officer for SHP Pilot will be deployed in one of two selected cities. The officer will be contracted by USAID – TBPS project. Day to day work of the officers will be in coordination with USAID – HFA and TBPS project.
Scope of Work
Key activities Technical Officer for SHP Pilot at each District*
Lead outreach activities to key stakeholders at the district level associate with pilot preparation and pilot implementation
Facilitate project engagement with key stakeholder groups, including professional associations, health facility organizations, local government officials, etc.
Provide technical and coordination support to SHP team including meeting organization, drafting agendas and preparing materials as needed
Gather relevant information during the technical document development of a Strategic Health Purchasing pilot for TB health services;
Liaise with partner organizations and project counterparts to coordinate, align, and build on analytical and technical input to SHP processes and health financing objectives
Support pilot implementation activities including training and monitoring
Assess sub-national (district and sub district) capacity and needs for improving enabling factors needed for pilot implementation (e.g. strengthen monitoring mechanism of providers, improving quality service delivery by improving providers capacity, strengthening planning referral mechanism, and improving providers capacity in case finding and notification);
Provide support to key stakeholders (e.g. providers, Dinkes, BPJSK, etc) to improve education, quality of services, and networking of providers as required by the design of the pilot;
Establish routine checks and feedback mechanisms to assess understanding and address challenges of implementers
Communicate pilot implementation results orally and in writing to a variety of audiences
Contribute in district-level project management activities during the pilot preparation, pilot implementation, and end-phase of the pilot
Coordinate with local stakeholders, develop the plan of actions for achieving milestones at the district/sub-district level, and ensure the readiness of the enabling factors for district activities
Participate in the development of roles and responsibilities tool and make sure that each stakeholder is accountable
Develop activity-reports based on the district-level activities (periodic report and per-activity/incidental report)
Facilitate research activities
Data collection and data analysis (for situational analysis, Budget Impact Analysis, paper-based payment simulation)
Arrange research permission and ethical clearance from local stakeholders
Tariff/ market price survey
Conducting other ad-hoc studies (qualitative and quantitative studies, survey, interview) related to pilot preparation and pilot implementation

Key Qualifications
Clinical and/or public health background - degree in medicine, public health, nursing, pharmacy, health economics or health policy
5-7 years of direct work experience related to health financing fields
Fluency in oral and written English and Bahasa Indonesia
Sound knowledge of the Indonesian health system, Ministry of Health, BPJS-K, DJSN, or other health sector.
Analytical, problem solving, and quantitative skills, with experience in analytical tools for health financing (costing, expenditure tracking, etc.).
Research experience with data collection and leading FGDs and in-depth interviews
Ability to conduct virtual meetings through online platforms and facilitating virtual discussion
Strong experience in design and implementation of aid development program at sub national level is desirable
Experience in building stakeholder relationships and facilitating decision-making at district level to create positive change
Strong experience in facilitating technical workshops, including developing term of references (TOR), agenda and background materials, presenting to and moderating discussion
Ability to work independently and collaboratively as part of a team and deliver high- quality results within tight timeframes.
Willingness to travel (up to 25% domestic travel)
