Introduction
We are seeking a highly motivated and skilled Assistant Pharmacist to join our dynamic team in a fast‑paced hospital pharmacy environment. Responsibilities
- Assist the lead pharmacist in preparing, dispensing, and verifying prescription medications for in‑patients and out‑patients.
- Maintain accurate inventory records, conduct regular stock checks, and coordinate with suppliers to ensure timely replenishment of medicines and medical supplies.
- Compounding sterile and non‑sterile preparations under strict aseptic techniques, following hospital protocols and safety guidelines.
- Provide clear and concise medication counseling to patients and caregivers, addressing dosage, administration routes, possible side effects, and storage instructions.
- Document all pharmacy activities in the electronic health record system, ensuring traceability and compliance with national pharmacy regulations.
- Participate in medication reconciliation processes during patient admissions, transfers, and discharges to prevent drug‑related problems.
- Collaborate with multidisciplinary healthcare teams—including physicians, nurses, and laboratory staff—to optimize therapeutic outcomes.
- Support quality‑assurance initiatives, such as conducting temperature monitoring, performing routine equipment calibration, and assisting in internal audits.
- Respond promptly and effectively to emergency situations, including rapid drug preparation for code blue or trauma cases.
- Adhere to infection‑control policies, maintain a clean and organized work environment, and handle hazardous substances according to safety standards.
Requirements
- Minimum education: High school diploma (SMA/SMK) or equivalent; additional pharmacy‑related training or certification is a strong advantage.
