Introduction
We are seeking a dedicated Tenaga Vokasi Kefarmasian (TVK) – Praktisi Farmasi to join our dynamic pharmacy team. This role is integral to delivering safe, effective, and patient‑centered pharmaceutical services within a busy hospital environment. The successful candidate will work closely with physicians, nurses, and allied health professionals to ensure optimal medication therapy outcomes for our diverse patient population.
Responsibilities
- Verify and dispense prescription orders accurately, adhering to hospital protocols and national regulatory standards.
- Provide medication counseling and education to patients and caregivers, ensuring they understand dosage, administration, potential side effects, and storage requirements.
- Prepare and compound sterile and non‑sterile pharmaceutical products, including IV admixtures, following aseptic techniques and quality‑control checks.
- Maintain and manage pharmacy inventory, conduct regular stock reconciliations, and coordinate with suppliers to prevent drug shortages.
- Collaborate with the multidisciplinary healthcare team during ward rounds, offering drug‑information support and therapeutic recommendations.
- Monitor medication therapy outcomes, identify drug‑related problems, and propose interventions to improve patient safety.
- Ensure compliance with all applicable laws, regulations, and hospital policies, including proper documentation of controlled substances.
- Participate in continuous quality improvement initiatives, audits, and pharmacy accreditation activities.
- Assist in training pharmacy interns, junior staff, and other healthcare personnel on best practices in medication management.
- Respond promptly to emergency drug requests and provide critical care medication support during code situations.
Requirements
- Minimum education: D3 Pharmacy (or equivalent). Candidates with at least a high school diploma (SMA/SMK) are also considered for entry‑level training.
- Valid Surat Tanda Registrasi (STR) and a current KARTANU or equivalent recommendation from the Nahdlatul Ulama (NU) organization.
- Age not exceeding 35 years at the time of application.

- Excellent communication skills in Bahasa Indonesia and the ability to convey complex pharmaceutical information clearly.
- Proficiency with pharmacy management software and basic computer literacy.
