Introduction
We are seeking a highly motivated Senior Pharmacist – Hospital Pharmacy Management to join our dedicated team in a dynamic, patient‑centered hospital environment. This senior role is pivotal for ensuring the safe, effective, and efficient delivery of pharmaceutical services across all clinical units. The successful candidate will lead a multidisciplinary pharmacy team, oversee medication therapy management, and support continuous improvement initiatives that enhance patient outcomes and operational excellence.
Responsibilities
- Supervise daily pharmacy operations, including medication dispensing, inventory control, and compliance with national regulations.
- Develop and implement standard operating procedures (SOPs) for drug procurement, storage, and distribution to maintain optimal safety and cost‑effectiveness.
- Collaborate with physicians, nurses, and allied health professionals to provide evidence‑based medication therapy recommendations.
- Conduct medication reconciliation for admitted, transferred, and discharged patients, reducing medication errors and adverse drug events.
- Lead and mentor junior pharmacists and pharmacy technicians, fostering a culture of continuous learning and professional development.
- Monitor and analyze pharmacy performance metrics, preparing regular reports for senior management.
- Ensure compliance with the Surat Tanda Registrasi (STR) requirements and other licensing standards.
- Participate in hospital committees related to drug safety, formulary management, and infection control.
- Manage shift schedules, ensuring adequate coverage for 24‑hour pharmacy services.
- Oversee the preparation and dispensing of high‑alert medications, chemotherapy, and sterile compounding in accordance with aseptic techniques.
- Stay current with emerging pharmaceutical therapies, clinical guidelines, and regulatory changes to advise the organization appropriately.
Requirements
- Minimum education: D3 or D4 in Pharmacy (Farmasi) from an accredited institution.
- Valid STR (Surat Tanda Registrasi) registration.
- At least 1 year of experience in a hospital pharmacy setting; experience in a senior or supervisory role is preferred.
- Strong commitment to patient safety, ethical practice, and high‑quality service delivery.
- Excellent teamwork and communication skills, with the ability to collaborate across multidisciplinary teams.
- Willingness to work rotating shifts, including evenings and weekends as required.
- Proficiency in pharmacy management software and basic computer applications.
- Demonstrated problem‑solving abilities, attention to detail, and organizational competence.
