Introduction
We are seeking a dedicated Spesialis Layanan Farmasi Rumah Sakit to join our dynamic pharmacy team at a leading regional hospital in Kediri. This role is pivotal in ensuring safe, effective, and timely medication therapy for patients across all departments. The ideal candidate will combine solid pharmaceutical knowledge with a compassionate approach to patient care, supporting both inpatient and outpatient services while adhering to the highest standards of clinical governance.
Responsibilities
- Prepare, dispense, and verify prescription orders for in‑patient and out‑patient settings, ensuring accuracy and compliance with hospital protocols.
- Provide medication counseling to patients and their families, explaining dosage, administration routes, potential side effects, and storage requirements.
- Manage inventory of pharmaceutical products, including stock rotation, expiration monitoring, and coordination with suppliers to maintain optimal stock levels.
- Participate in multidisciplinary rounds, offering drug‑therapy recommendations and collaborating with physicians, nurses, and allied health professionals.
- Conduct medication reconciliation upon admission, transfer, and discharge to prevent therapeutic duplications or omissions.
- Support the hospital’s antimicrobial stewardship program by reviewing antibiotic usage and promoting evidence‑based prescribing.
- Ensure compliance with national regulations, hospital policies, and Good Pharmacy Practice (GPP) standards, including accurate documentation of controlled substances.
- Assist in the preparation of compounded sterile preparations (CSP) under aseptic conditions when required.
- Participate in continuous quality improvement initiatives, audit processes, and adverse drug event reporting.
- Provide training and mentorship to pharmacy interns, junior staff, and other healthcare workers on medication safety and best practices.
Requirements
- Education: Minimum D3/D4 in Pharmacy (DIll Farmasi) from an accredited institution.
- Licensure: Active STR (Surat Tanda Registrasi) required.
- Experience: At least 1 year of relevant experience in a hospital pharmacy setting; fresh graduates are welcome to apply.
- Age: Maximum 30 years.
- Gender: Open to all genders; candidates must be of the Islamic faith as per institutional policy.
- Residency: Preference given to candidates residing in Kediri or surrounding areas.
- Shift Work: Willingness to work a rotating shift schedule, including weekends (Monday‑Saturday).
