Responsibilities
- Prepare, dispense, and verify prescription medications in accordance with hospital protocols and national regulations.
- Conduct medication therapy management, providing counseling and education to patients, physicians, and nursing staff.
- Maintain accurate and up‑to‑date medication records in the hospital’s electronic health record (EHR) system.
- Monitor drug interactions, dosage appropriateness, and adverse drug reactions, intervening promptly when issues arise.
- Collaborate with physicians, nurses, and other healthcare professionals to develop individualized treatment plans.
- Manage inventory, perform regular stock checks, and coordinate with suppliers to ensure availability of essential drugs.
- Participate in clinical rounds, offering pharmacological insights and recommendations.
- Ensure compliance with all regulatory bodies, including maintaining an active Surat Tanda Registrasi (STR) and any additional professional certifications.
- Provide mentorship and support to junior pharmacy staff and pharmacy students during their clinical rotations.
- Contribute to continuous quality improvement initiatives, including medication safety audits and policy development.
Requirements
- Education: Minimum S1 in Pharmacy (SI Profesi Apoteker / Apt).
- Professional License: Active STR (Surat Tanda Registrasi) as a pharmacist.
- Experience: Minimum 2 years of experience in a hospital or clinical pharmacy setting.
- Age: Must be 28 years old or younger.
- Gender: Open to all genders.
- Additional Certifications: Any supporting professional certificates (e.g., clinical pharmacy, drug information).
